Subendocardial and Transmural Myocardial Ischemia Clinical Characteristics, Prevalence, and Outcomes With and Without Revascularization

Summary: This study aimed to quantify subendocardial ischemia and assess its clinical associations and outcomes. The results showed that severely reduced coronary flow capacity (CFC) was associated with a higher risk of major adverse cardiac events and mortality. For patients without severe CFC abnormality, although they had subendocardial perfusion defects, the risk of adverse cardiac events was relatively low.

The impact of coronary revascularization on vessel-specific coronary flow capacity and long-term outcomes: a serial [15O]H2O positron emission tomography perfusion imaging study

Summary: This study evaluated the effects of revascularization on coronary flow capacity (CFC) using [O-15]H2O PET perfusion imaging. The results showed that successful revascularization leads to an increase in CFC, and baseline CFC is an independent predictor of changes in myocardial blood flow and coronary flow reserve. Moreover, an increase in CFC is associated with a favorable outcome in terms of death and non-fatal myocardial infarction.

Defining the prognostic value of [15O]H2O positron emission tomography-derived myocardial ischaemic burden

Summary: This study aimed to define a prognostic threshold for the myocardial ischaemic burden (IB) derived from [O-15]H2O positron emission tomography (PET). A hyperaemic myocardial blood flow (hMBF) IB of 24% and myocardial flow reserve (MFR) IB of 28% were identified as prognostic thresholds. Patients with high hMBF or MFR IB had worse outcomes compared to those with low IB levels.

Mortality Prediction by Quantitative PET Perfusion Expressed as Coronary Flow Capacity With and Without Revascularization

Summary: This study aimed to explore the relationship between reduced quantitative perfusion parameters and mortality with and without revascularization. It found that severely reduced CFR and stress perfusion incurred increasing size-dependent risks, with severely reduced CFC associated with the highest major adverse cardiac event rate and mortality risk. Small regions of CFCsevere predicted high risk due to border zones at risk around the severe center, and this risk could be reduced by revascularization.
