Premature atrial contractions promote local directional heterogeneities in conduction velocity vectors

Summary: Loss of cell-to-cell communication leads to local conduction disorders and directional heterogeneity (LDH) in conduction velocity (CV) vectors, which can be revealed by premature atrial contractions (PACs). This study quantified LDH and compared the differences between sinus rhythm (SR) and PACs in patients with and without atrial fibrillation (AF). The results showed that LDH increased during PACs, especially at Bachmann's bundle (BB) and pulmonary vein area (PVA). Patients with AF already had more LDH during SR, and this heterogeneity became more pronounced during PACs.

Elevated fibrosis burden as assessed by MRI predicts cryoballoon ablation failure

Summary: In this study, we evaluated the usefulness of LGE-MRI-based fibrosis quantification in predicting arrhythmia recurrence after cryoballoon ablation. The results showed that fibrosis quantification using the PIH method was able to predict recurrence, while the IIR method was not. There was a modest overlap between the designated fibrosis areas by the two methods. Therefore, caution should be exercised when interpreting LA fibrosis from LGE-MRI, as the values and spatial patterns depend on the methodology.

Summary: Despite advances in interventional treatment strategies, atrial fibrillation remains associated with significant morbidity and mortality. Fibrotic atrial myopathy is a main factor for adverse outcomes of AF-ablation and a new risk stratification model termed AF-SCORE has been derived using easily available clinical parameters to predict FAM and ablation-success. Age, sex, and AF-phenotype are the main determinants for the development of FAM, with a low AF-SCORE <= 2 associated with favorable outcomes of PVI-only.

Identification of local atrial conduction heterogeneities using high-density conduction velocity estimation

Summary: Accurate determination of intra-atrial conduction velocity is crucial for identifying arrhythmogenic areas. This study aimed to evaluate different methodologies for measuring conduction heterogeneity and found that the DVV technique was most accurate in identifying local areas of conduction heterogeneities, while PSF and FiD techniques smoothed wavefront propagation. Additionally, patients with paroxysmal atrial fibrillation showed slower wavefront propagation at Bachmann's bundle during sinus rhythm, indicating structural remodelling.
