Improved transition to adult care in youth with type 1 diabetes: a pragmatic clinical trial

Summary: Youth with type 1 diabetes are at high risk for loss to follow-up during the transition from paediatric to adult diabetes care. An intervention using communication technology enhanced transition coordinator has been shown to improve clinic attendance among transitioning youth with type 1 diabetes, with no differences in glycaemic control or diabetic ketoacidosis post transfer.

Improving Adolescent and Young Adult Engagement in the Process of Transitioning to Adult Care

Summary: The purpose of this project was to improve engagement of adolescents and young adults, as well as parents/caregivers, in the process of transitioning from pediatric to adult care using clinical readiness assessment tools and the Institute for Healthcare Improvement Model. Results showed improved readiness assessment scores among participants, as well as some improvements in the implementation of transition policies at the public health center, although patient portal enrollment did not meet the expected goal.
