The role of cardiovascular magnetic resonance in the evaluation of acute myocarditis and inflammatory cardiomyopathies in clinical practice - a comprehensive review

Tevfik F. Ismail et al.

Summary: Inflammatory cardiomyopathy is myocarditis associated with cardiac dysfunction and ventricular remodeling, with various infectious and non-infectious causes. Genetic, autoimmune, and environmental factors contribute to the risk of worsening cardiac function and other complications. Multi-parametric cardiovascular magnetic resonance is a useful tool for non-invasive diagnosis.

Summary: This study aimed to develop and validate revised classification criteria for EGPA. Through the identification, collection, and analysis of candidate criteria items, a risk score system was established for disease classification in EGPA patients and comparator diseases. The criteria demonstrated strong sensitivity and specificity after validation.

Summary: The study developed and validated revised classification criteria for granulomatosis with polyangiitis (GPA), identifying final criteria and weights through regression analysis. In the validation dataset, the criteria demonstrated strong performance characteristics with 93% sensitivity and 94% specificity, making them suitable for research purposes.

Cardiovascular magnetic resonance in autoimmune rheumatic diseases: a clinical consensus document by the European Association of Cardiovascular Imaging

S. Mavrogeni et al.

Summary: Patients with autoimmune rheumatic diseases (ARDs) often have cardiovascular diseases (CVD) and a reduced life expectancy. Currently used evaluation methods may delay treatment opportunities, while cardiovascular magnetic resonance (CMR) can identify structural and functional changes in the disease early before clinical manifestations appear, providing an accurate assessment of disease activity. Therefore, CMR is of great importance for timely clinical decision-making and personalized treatment.

Prevalence and prognostic significance of heart failure with preserved ejection fraction in systemic sclerosis

Marta Fontes Oliveira et al.

Summary: This study examined the prevalence of heart failure with preserved ejection fraction (HFpEF) in systemic sclerosis (SSc) patients, finding that one in four patients met the HFpEF criteria. These HFpEF patients were older, had more cardiovascular risk factors, and were more likely to have arrhythmias or lung disease. Although SSc patients with HFpEF had a 3.4-fold increased risk of dying over 9 years, this association lost statistical significance after adjusting for age, while NT-proBNP remained an independent predictor of worse prognosis.

Summary: In this study, 25% of EGPA patients were found to have cardiac specific involvement, with common manifestations including myocarditis, regional wall motion abnormalities, and apical aneurysm. Patients with CSI had higher peak eosinophil count at diagnosis.

Summary: The study found that 45% of asymptomatic patients had abnormal baseline cardiac evaluations, aiding in the earlier detection of cardiac diseases. It is recommended to conduct prompt cardiac screening for all EGPA patients instead of following a symptoms-guided algorithm.
