Epidemiology, Natural History, and Outcomes of Primary Sclerosing Cholangitis: A Systematic Review of Population-based Studies

Epidemiology and outcomes of primary sclerosing cholangitis: an Australian multicentre retrospective cohort study

Summary: This study evaluated the epidemiology and clinical outcomes of primary sclerosing cholangitis (PSC) in Australia and compared them to the general population. The results showed that PSC patients had a higher risk of developing cirrhosis and death, especially from cholangiocarcinoma. Age and time from diagnosis were closely associated with disease severity and prognosis.

The analysis found a positive causal effect of IBD on PSC, providing implications for prevention and treatment of both diseases. Further analyses revealed statistical heterogeneity and sensitivity of the single nucleotide polymorphisms (SNPs) used in the study.

Summary: Primary sclerosing cholangitis (PSC) is a rare chronic liver disease characterized by bile duct inflammation, fibrosis, and cirrhosis, often associated with inflammatory bowel disease (IBD) and diagnosed in adulthood. Currently, there is no effective medical therapy for PSC, and management focuses on symptom alleviation and complication prevention. Liver transplantation (LT) remains the only life-extending treatment for end-stage PSC, with a high rate of recurrence and post-transplant morbidity.
