Dissemination of NDM-producing bacteria in Southern Brazil

Summary: This study investigated carbapenem-resistant isolates from multiple hospitals in Southern Brazil and identified 486 bacteria carrying the NDM gene. The incidence of NDM-producing bacteria varied, with the most common strain being K. pneumoniae. One K. pneumoniae strain carried 5 plasmids: Col440I, Col440II, IncFIA(HI1), IncFIB(K), IncFIB(pQil)/IncFII(K), and IncR. The presence of multiple plasmids may contribute to the expansion of NDM-producing bacteria.

Trend analysis of carbapenem-resistant Gram-negative bacteria and antimicrobial consumption in the post- COVID-19 era: an extra challenge for healthcare institutions

Summary: This study analyzed the incidence density trend of carbapenem-resistant Gram-negative bacteria and antimicrobial consumption in 99 critical care facilities in a low/middle-income country. The findings showed an increase in the incidence density of carbapenem-resistant Acinetobacter baumannii (CRAB) in 2020, which was strongly correlated with the incidence density of COVID-19. There was also an increase in polymyxin consumption in 2020, but it was not significantly correlated with CRAB or COVID-19 incidence density, likely due to empirical and untargeted prescribing.

Description of a novel IncP plasmid harboring blaKPC-2 recovered from a SPM-1-producing Pseudomonas aeruginosa from ST277

Summary: The high rates of carbapenem resistance among Brazilian Pseudomonas aeruginosa isolates are mainly associated with the clone ST277 producing the carbapenemase SPM-1. The complete genetic composition of a IncP plasmid harboring blaKPC-2 in isolates of this endemic clone carrying chromosomal blaSPM-1 was described using whole genome sequencing. These results confirm the association of these two carbapenemases in ST277 and also describe the genetic composition of a novel blaKPC-2-plasmid. Considering the fact that this association occurs in a high-risk clone, monitoring the dissemination of this plasmid should be a public health concern.
