Harnessing the potential of practice-based clinical optometry research in the United Kingdom

Laura J. J. Taylor et al.

Summary: Research is crucial for evidence-based practice in healthcare, including optometry. The College of Optometrists in the UK sets national standards for optometric practice, but there is currently limited evidence directly related to optometry. Government initiatives aim to increase optometrists' engagement in research and promote collaboration among stakeholders.

Inherited retinal diseases: Linking genes, disease-causing variants, and relevant therapeutic modalities

Nina Schneider et al.

Summary: Inherited retinal diseases (IRDs) are a complex and heterogeneous group of visual impairments caused by pathogenic variants in numerous genes. The understanding of the genotype-phenotype correlations, inheritance patterns, hypomorphic alleles, and modifier genes associated with IRDs is challenging due to their unique characteristics. Advances in next-generation sequencing have facilitated the identification of novel IRD-related genes and variants. The Global Retinal Inherited Disease (GRID) dataset provides valuable information on the frequencies and complexities of pathogenic variants among 194 genes.

Measurement Properties of the Attitudes to Gene Therapy for the Eye (AGT-Eye) Instrument for People With Inherited Retinal Diseases

Myra B. McGuinness et al.

Summary: The study aimed to assess the measurement properties of the AGT-Eye instrument among Australian adults with inherited retinal diseases (IRDs) and parents/caregivers of people with IRDs. The results showed that the four subscales of the AGT-Eye could be used to analyze operational knowledge and perceived value of ocular gene therapy in Australia, but further improvement in measurement properties is needed.

Genetic Testing of Inherited Retinal Disease in Australian Private Tertiary Ophthalmology Practice

Sena A. Gocuk et al.

Summary: The prevalence of genetic testing for inherited retinal diseases (IRDs) in a tertiary practice setting was assessed in this study. The results showed a low uptake of testing (around 10%) in the Australian private practice IRD cohort, reflecting historical management patterns and accessibility of genetic counselling and testing. Younger patients and those with a longer duration of care were more likely to have received genetic testing.

Summary: In the study "Background Retinitis pigmentosa", telephone interviews were conducted with RP patients who underwent genetic testing. The results showed that RP patients were generally satisfied with their testing experience, despite having different motivations and expectations for the test.

An optometrist's guide to the top candidate inherited retinal diseases for gene therapy

Fleur O'Hare et al.

Summary: This review presents the phenotypic and genotypic profiles of a select group of inherited retinal diseases (IRDs) that are currently the focus of retinal gene therapy trials globally. It highlights the salient clinical characteristics of retinitis pigmentosa, the largest IRD category, as well as specific IRDs such as Leber congenital amaurosis, choroideremia, and Stargardt disease. Guidance on clinical diagnostic tests and monitoring IRD progression are provided for optometrists, to assist in assessment and management of individuals eligible for gene therapies.

Functional expression of complement factor I following AAV-mediated gene delivery in the retina of mice and human cells

Anna K. Dreismann et al.

Summary: This study demonstrates the potential for treating AMD by supplementing Complement Factor I (CFI) through AAV vectors. Codon-optimised constructs for expressing CFI showed reduced secretion compared to non-optimised sequences, but still maintained functional activity. In vivo expression analysis showed predominant expression of CFI in RPE and photoreceptors, with actively functioning secreted protein in vitreous humour.

Genetic testing and diagnosis of inherited retinal diseases

Byron L. Lam et al.

Summary: Inherited retinal diseases are a diverse group of degenerative diseases that can cause significant vision loss and blindness. Genetic testing is crucial for obtaining a definitive diagnosis and accessing gene therapy-based treatments, but there is limited training in genetic diagnostics among ophthalmologists. Advances in molecular diagnostics have improved understanding and identification of disease-causing variants in IRDs.

Summary: This study aims to develop a tool to assess knowledge, attitudes, and perceptions of approved and future genetic therapies for ocular conditions. It also aims to evaluate quality of life, attitudes towards clinical trials, and vision-related quality of life among potential recipients of ocular gene therapy. The study plans to recruit 500 Australian patients for an internet-based survey and is expected to be published in early 2021.
