Assessing the arrhythmogenic risk of engineered heart tissue patches through in silico application on infarcted ventricle models

Damiano Fassina et al.

Summary: The presence of fibrotic tissue and disrupted electrical properties in post-MI ventricles increases the risk of life-threatening arrhythmias. Epicardial patches derived from hiPSC-CMs are a potential therapy for post-MI heart failure, but their effects on arrhythmias have not been studied. This study used in silico models to investigate the impact of EHT patch engraftment on arrhythmic risk.

Myofibroblast senescence promotes arrhythmogenic remodeling in the aged infarcted rabbit heart

Brett C. Baggett et al.

Summary: Progressive tissue remodeling after myocardial infarction promotes cardiac arrhythmias. Little is known about pro-arrhythmic changes in aged animals. In this study, aged rabbits exhibited increased peri-procedural mortality and arrhythmogenic electrophysiological remodeling at the infarct border zone (IBZ) compared to young rabbits. Computational modeling showed that senescent myofibroblast-cardiomyocyte coupling prolongs action potential duration (APD) and facilitates conduction block permissive of arrhythmias. The findings suggest that therapeutic interventions targeting senescent cells may mitigate arrhythmias post-MI with age.

Cardiac Fibroblasts and Myocardial Regeneration

Wangping Chen et al.

Summary: The loss of cardiomyocytes in acute myocardial infarction cannot be effectively replaced by limited regenerative capacity of adult mammalian hearts. Cardiac fibroblasts play a crucial role in fibrosis after MI and may hold potential for reprogramming into cardiomyocytes. Further research is needed to explore the potential of cardiac fibroblasts in improving cardiac performance in injured hearts.

Pharmacologic therapy for engraftment arrhythmia induced by transplantation of human cardiomyocytes

Kenta Nakamura et al.

Summary: Heart failure remains a significant cause of morbidity and mortality following myocardial infarction. Cardiac remuscularization with transplantation of human pluripotent stem cell-derived cardiomyocytes is a promising preclinical therapy to restore function. Recent large animal data, however, have revealed a significant risk of engraftment arrhythmia (EA). Pharmacologic treatment of EA may be a viable strategy to improve safety and allow further clinical development of cardiac remuscularization therapy.

Intramyocardial Transplantation of Human iPS Cell-Derived Cardiac Spheroids Improves Cardiac Function in Heart Failure Animals

Shinji Kawaguchi et al.

Summary: The severe shortage of donor hearts hampers cardiac transplantation, leading to high expectations for cardiac regenerative therapies. hiPSC-derived cardiomyocytes offer a realistic cell source for heart recovery. Avoiding teratoma formation and scaling up cardiomyocyte culture are essential for clinical applications. Spheroids and gelatin hydrogel combination has shown success in recovering reduced ejection fraction, and a feasible transplantation strategy for regenerative cardiomyocytes has been established in this study.
