Summary: The ESC 0/1-hour algorithm is effective in diagnosing acute coronary syndrome, but limited data is available for patients with known coronary artery disease. This study evaluated and compared the diagnostic performance of the algorithm for cardiac death or myocardial infarction in patients with and without known CAD, and assessed its ability to achieve a negative predictive value of 99% or higher.

Summary: This study evaluated the diagnostic accuracy of a high-sensitivity cardiac troponin I assay in the Emergency Department. Absolute delta criteria showed superior accuracy compared to relative criteria. Using the Siemens TNIH assay in combination with validated algorithms incorporating specific cutoff values and absolute delta criteria can improve sensitivity in diagnosing acute myocardial infarction.

Acute coronary syndrome treatment delay in low to middle-income countries: A systematic review

Summary: Morbidity and mortality rates for acute coronary syndrome (ACS) are decreasing in high-income countries but rising at an alarming pace in low to middle income countries (LMICs). Factors contributing to poor clinical outcomes in LMICs include prehospital treatment delays, with older age, female gender, illiteracy, rural residence, and financial limitations associated with longer delays. Lack of developed emergency transportation systems and poor communication between community and interventional facilities are also major contributors to delays in ACS treatment.
