Summary: This review examines the role of folate, particularly its synthetic and bioactive forms, in modifying metabolic outcomes in mothers and offspring during pregnancy. An unfavorable in utero environment may have long-lasting effects on cellular and physiological functions, affecting the development of the child and the postpartum health of the mother. Overconsumption of synthetic folic acid (FA) by pregnant women in North American countries raises concerns about its potential unintended health effects. Clinical studies comparing the effects of FA and the bioactive form of folate, 5-methyltetrahydrofolic acid, during pregnancy are limited. Caution is advised when maternal intake of either form of folate exceeds recommended amounts, and more research is urgently needed to determine the optimal dose and form of folate for healthy pregnancy and long-term metabolic outcomes in mothers and children.
