Efficacy and tolerability of pharmacological treatments for insomnia in adults: A systematic review and network meta-analysis

Summary: Insomnia is a common and burdensome disorder in adults. We compared and ranked the efficacy and tolerability of insomnia medication. 69 studies involving 17,319 patients were analyzed, and we found that orexin receptor antagonists (ORAs) were more effective and tolerable than benzodiazepine-like drugs (Z-drugs) and melatonin receptor agonists (MRAs) for insomnia treatment. The findings provide guidance for clinicians in choosing appropriate medications for insomnia symptoms.

Summary: The study found that participants who received suvorexant after exposure therapy in the evening had increased N3 sleep and decreased N2 sleep and rapid eye movement latency. Although there was greater habituation between exposure sessions with suvorexant, there was no significant difference in the reduction of PTSD severity from baseline to 1-week follow-up between groups. Further research with larger samples is needed to determine the clinical applications of enhancing exposure therapy with suvorexant.

The study shows that suvorexant, which increases REM sleep, enhances fear extinction compared to zolpidem, which decreases REM sleep.
