Ketamine as advanced second-line treatment in benzodiazepine-refractory convulsive status epilepticus in children

Summary: Status epilepticus (SE) is a common neurological emergency in children with no definitive evidence for the treatment of refractory SE. Protocols differ among institutions, but valproate, phenytoin, and levetiracetam are frequently used as second-line treatment. Ketamine, a noncompetitive N-methyl-D-aspartate receptor antagonist, has shown potential as an advanced second-line agent for the treatment of SE. Future research should focus on the role of ketamine as second-line medication in SE pharmacotherapy.

Chemogenetic attenuation of cortical seizures in nonhuman primates

Summary: Pharmacological and surgical treatments of epilepsy have limited success, therefore a more targeted and on-demand approach is desired. The authors demonstrate the use of inhibitory chemogenetics to attenuate cortical seizures and subsequent convulsions in nonhuman primates. These results highlight the efficacy of DREADDs for treating epilepsy in a primate model.

Summary: Recent clinical studies suggest that exposure to general anaesthetics during infancy may lead to long-term behavioural alterations rather than cognitive impairments. Preclinical studies, especially in nonhuman primates, provide evidence of behavioural changes but cognitive impairments are still controversial. Strategies such as co-administration of dexmedetomidine with anaesthetic drugs or alternative use of hypnotic neurosteroids are being explored to protect against anaesthesia-induced developmental neurotoxicity.
