Long-Term Follow-up of Enhanced Holter-Electrocardiography Monitoring in Acute Ischemic Stroke

Rolf Wachter et al.

Summary: Prolonged electrocardiography monitoring in stroke patients improves the detection of paroxysmal atrial fibrillation, especially during the first six months. However, long-term follow-up showed a higher incidence of clinical atrial fibrillation in the usual-care group.

Prolonged Cardiac Monitoring and Stroke Recurrence A Meta-analysis

Georgios Tsivgoulis et al.

Summary: Analysis of 8 studies (5 RCTs, 3 observational) revealed that patients receiving PCM after IS or TIA had better AF detection rates and anticoagulant initiation rates. However, there is no solid RCT evidence supporting that PCM may be associated with lower stroke recurrence risk.

Global, regional, and national burden of stroke and its risk factors, 1990-2019: a systematic analysis for the Global Burden of Disease Study 2019

Valery L. Feigin et al.

Summary: Regularly updated data on stroke including incidence, prevalence, mortality, disability, risk factors, and trends are crucial for evidence-based care planning. The Global Burden of Diseases Study provides comprehensive measurement of these metrics. From 1990 to 2019, while age-standardized rates improved, the number of strokes and deaths increased. Low-income countries had the highest stroke-related mortality and DALY rates, with high body-mass index being the fastest-growing risk factor.

Effect of Long-term Continuous Cardiac Monitoring vs Usual Care on Detection of Atrial Fibrillation in Patients With Stroke Attributed to Large- or Small-Vessel Disease The STROKE-AF Randomized Clinical Trial

Richard A. Bernstein et al.

Summary: The STROKE-AF trial demonstrated that among patients with stroke attributed to large- or small-vessel disease, long-term cardiac monitoring with an insertable cardiac monitor (ICM) detected significantly more cases of atrial fibrillation (AF) over 12 months compared to usual care. However, further research is needed to determine the clinical significance of identifying AF in these patients.

Effect of Implantable vs Prolonged External Electrocardiographic Monitoring on Atrial Fibrillation Detection in Patients With Ischemic Stroke The PER DIEM Randomized Clinical Trial

Brian H. Buck et al.

Summary: This study compared the detection of atrial fibrillation in patients with recent ischemic stroke using implantable loop recorder versus external loop recorder, and found that the implantable loop recorder group had a significantly higher proportion of AF detected over 12 months. Further research is needed to compare clinical outcomes and cost-effectiveness of these monitoring strategies.

Systematic monitoring for detection of atrial fibrillation in patients with acute ischaemic stroke (MonDAFIS): a randomised, open-label, multicentre study

Karl Georg Haeusler et al.

Summary: Systematic ECG monitoring can increase the detection rate of atrial fibrillation in patients hospitalized with acute ischemic stroke or transient ischemic attack, but it does not significantly affect the rate of oral anticoagulant use after 12 months. Further efforts are needed to improve secondary stroke prevention.
