Metachronous gastric neoplasm beyond 5 years after endoscopic resection for early gastric cancer

Eunwoo Lee et al.

Summary: This study aimed to clarify the long-term clinical outcomes and risk factors of metachronous gastric neoplasm (MGN) 5 years after endoscopic submucosal dissection (ESD) for early gastric cancer (EGC). The study found that male sex, severe intestinal metaplasia, tumor-positive lateral margin, <5 mm lateral safety margin, and synchronous adenoma were positive predictive factors for MGN after 5 years post-ESD. In addition, successful eradication of Helicobacter pylori was a negative predictive factor for MGN.

Effect of Helicobacter pylori Eradication Therapy on the Incidence of Noncardia Gastric Adenocarcinoma in a Large Diverse Population in the United States

Dan Li et al.

Summary: This large-scale retrospective cohort study in the United States investigated the incidence of noncardia gastric adenocarcinoma (NCGA) after Helicobacter pylori eradication therapy. The findings showed a significantly reduced risk of NCGA after 8 years of treatment compared to no treatment. This study highlights the potential for substantial gastric cancer prevention through H pylori eradication. Evaluation: 8 out of 10.

Summary: This study systematically searched and analyzed randomized and non-randomized studies to assess the risk of gastric cancer associated with the use of proton-pump inhibitors (PPIs). The results showed no association between PPIs and gastric cancer, especially based on non-randomized studies.

Risk factors for gastric metachronous lesions after endoscopic or surgical resection: a systematic review and meta-analysis

Raquel Ortigao et al.

Summary: This study conducted a systematic review of 52 studies to identify risk factors for metachronous gastric lesions (MGL) and compare the incidence after endoscopic resection (ER) and subtotal gastrectomy. The results showed that the incidence of MGL after ER was significantly higher than after subtotal gastrectomy. Factors such as age, sex, family history, synchronous lesions, gastric mucosal atrophy, intestinal metaplasia, H. pylori infection, and pepsinogen I/II ratio were significantly associated with MGL after ER. The study highlights the importance of individualized follow-up schedules based on different surgical procedures and risk factors.

Summary: This study investigated immortal time bias for a specific life-long condition, intellectual disability, using the Clinical Practice Research Datalink (CPRD). The results show that immortal time bias is a significant issue for studies of life-long conditions that use electronic health record data and careful consideration of clinical diagnoses entry is needed.

Use of Proton Pump Inhibitors and the Risk for the Development of Gastric Cancers: A Nationwide Population-Based Cohort Study Using Balanced Operational Definitions

Summary: Previous cohort studies using national claim data in Korea have shown conflicting results about the association between the use of proton pump inhibitors (PPIs) and the risk of gastric cancer. This study aimed to evaluate the association between PPI use and the risk of gastric cancer using balanced operational definitions, and found no increased risk.

Summary: This study aimed to evaluate the association between proton pump inhibitor (PPI) use and gastric cancer in a high-risk region like Korea. The results showed that PPI use was associated with an increased incidence of gastric cancer, regardless of Helicobacter pylori eradication status. Long-term use of PPI should be approached with caution in high-risk regions for gastric cancer.

When does proton pump inhibitor treatment become long term? A scoping review

Peter Fentz Haastrup et al.

Summary: The definition of long-term PPI treatment varied substantially between studies and were seldom rationalized. In a clinical context, the definition may vary depending on the patient's condition. For research purposes, a tailored definition may be necessary depending on the study's objectives. It is recommended to always rationalize the choice of definition.
