The evidence suggests some benefits of HIE in reducing readmission rates and mortality, but there is insufficient evidence for its impact on cost or utilization. The observational nature of the studies and the variability in outcomes limit the strength of the evidence.

Racial and Ethnic Differences in Hospice Use and Hospitalizations at End-of-Life Among Medicare Beneficiaries With Dementia

Summary: This cohort study explores the differences in end-of-life utilization of hospice and hospital services among patients with dementia by race and ethnicity. The results show unique patterns of end-of-life care utilization and treatment preferences among different racial and ethnic groups. Factors such as race, age, gender, education level, cognitive impairment, and daily living limitations are associated with the use of hospice care. Black and Hispanic beneficiaries with dementia are more likely to use emergency department and inpatient services before death, resulting in higher costs. The study highlights the importance of considering cultural competence in end-of-life care for patients with dementia.

Dementia is Associated With Poorer Quality of Care and Outcomes After Stroke: An Observational Study

Summary: The study found that patients with preexisting dementia receive poorer quality of care and have worse outcomes after stroke in the acute hospital phase. Patients with dementia were less likely to be treated in stroke units, receive timely therapy, and have access to rehabilitation services, and were more likely to be discharged to residential care.

Association Between Interhospital Care Fragmentation, Readmission Diagnosis, and Outcomes

Summary: This study assessed the impact of interhospital fragmentation on in-hospital mortality, length of stay, and costs in 30-day readmissions. The results showed that fragmented readmissions were associated with higher in-hospital mortality and cost. Mortality was consistently higher for fragmented readmissions for patients with congestive heart failure and chronic obstructive pulmonary disease.
