Summary: ABCC8 and KCJN11 mutations cause severe diazoxide-resistant congenital hyperinsulinism (CHI). Pasireotide, a second-generation somatostatin analogue, was used off-label in a boy with severe therapy-resistant CHI due to a homozygous ABCC8 mutation. While pasireotide treatment slightly improved glycemic control without side effects, it was not sufficient to prevent near-total pancreatectomy in this case.
