Summary: COVID-19 vaccines have brought hope against the pandemic, with some minor and transient neurological complications reported post-vaccination. The most devastating complication is cerebral venous sinus thrombosis, while Bell's palsy is another major concern. Additional unexpected adverse events include acute transverse myelitis, acute disseminated encephalomyelitis, and acute demyelinating polyneuropathy, as well as reactivation of herpes zoster. Studies are needed to explore the causal association between vaccines and these neurological events.

Summary: This study describes a case of ischemic stroke with large vessel occlusion (LVO) after Ad26.COV2.S vaccination, and systematically reviews published cases of ischemic stroke and vaccine-induced immune thrombotic thrombocytopenia (VITT) following COVID-19 vaccination. The results show that in addition to cerebral venous thrombosis, adenoviral vector-based vaccines also appear to have a risk of cerebral arterial thrombosis, and clinicians should be aware that ischemic stroke with LVO, although rare, could represent a clinical presentation of VITT.

Summary: The first case of hyperacute reversible encephalopathy following COVID-19 vaccination is described, with the patient presenting with acute onset encephalopathy characterized by agitation and confusion. High dosage steroid therapy led to rapid response and complete remission within 3 days from onset, indicating a potential association between the clinical manifestation and systemic/CSF cytokine hyperproduction responsive to steroid therapy. The occurrence of encephalopathy after vaccination may be attributed to an excessive innate immune response against the vaccine, particularly in predisposed patients susceptible to autoimmunity.
