Consensus-Based Nutrition Care Pathways for Hospital-to-Community Transitions and Older Adults in Primary and Community Care

Heather Keller et al.

Summary: This study developed evidence- and consensus-based nutrition care pathways for adult patients at risk for malnutrition transitioning from hospital to community, with input from diverse stakeholders. Survey results showed high ratings of >= 80% for relevance and importance of most nutrition care practices. These pathways offer flexible guidance for different primary care and community models.

The effectiveness of nutrition interventions in improving frailty and its associated constructs related to malnutrition and functional decline among community-dwelling older adults: A systematic review

Summary: Multifactorial interventions were more effective than nutrition intervention alone in improving frailty and physical performance. Protein-energy supplementation appeared to be effective only in malnourished older adults. The use of micronutrient supplementation for frailty and related outcomes remains inconclusive. Existing clinical guidelines align with evidence recommending comprehensive interventions to improve frailty.

A 3-year prospective cohort study of dietary patterns and frailty risk among community-dwelling older adults

Summary: This study investigated the association between dietary patterns and frailty index in community-dwelling Japanese older adults. The results showed that adherence to sugar and fat, as well as salt and pickles dietary patterns, were positively associated with frailty, while adherence to a protein-rich dietary pattern was negatively associated with frailty.

Impact of nutritional status according to GLIM criteria on the risk of incident frailty and mortality in community-dwelling older adults

L. Rodriguez-Manas et al.

Summary: This study aimed to assess the role of nutritional status in predicting frailty and mortality risk in Spanish community-dwelling older adults. The findings suggest that being at malnutrition risk is associated with a higher risk of frailty, while malnourishment is linked to increased mortality risk.

Ageing rate of older adults affects the factors associated with, and the determinants of malnutrition in the community: a systematic review and narrative synthesis

Laura A. Bardon et al.

Summary: This review critically examines the evidence for the most frequently reported sociodemographic factors and determinants of malnutrition in older adults, with differences identified according to rates of ageing. Factors associated with malnutrition from the food intake, social, and disease-related domains increase in severity in the accelerated ageing category. This study highlights the importance of understanding specific factors associated with malnutrition in older adults based on their rate of ageing to improve identification and prevention strategies.
