Summary: This study compared the outcomes of valve-in-valve transcatheter aortic valve implantation (ViV TAVI) with repeat surgical aortic valve replacement (SAVR) in patients with a failed aortic bioprosthetic valve. The results showed that ViV TAVI was associated with lower in-hospital mortality but higher rates of 30-day and 6-month all-cause readmission compared with repeat SAVR. However, there were no differences in the risk of in-hospital stroke, post-procedure pacemaker implantation, major adverse cardiac events (MACE), and mortality during readmission between the two groups.

Incidence and Predictors of Permanent Pacemaker Implantation after Transcatheter Aortic Valve Procedures: Data of The Netherlands Heart Registration (NHR)

Summary: This study retrospectively analyzed the data of 7489 TAVI patients collected by the Netherlands Heart Registration and found that the incidence rate of permanent PM implantation after TAVI remained stable over time. Several clinical and procedural factors were identified as independent predictors of post-TAVI PM, and further efforts are needed to reduce the occurrence of this important issue in high-risk patients.

Outcomes in Valve-in-Valve Transcatheter Aortic Valve Implantation

Summary: The use of valve-in-valve transcatheter aortic valve implantation (ViV-TAVI) is increasing, and this study evaluated clinical outcomes in patients with ViV-TAVI compared to patients with native valve TAVI (NV-TAVI). It was found that ViV-TAVI is a safe and effective treatment for failing bioprosthetic valves with mortality and stroke rates comparable to NV-TAVI for both valve types. The study also compared outcomes in patients with ViV-TAVI treated with self-expandable versus balloon-expandable valves.
