Locating Advance Care Planning Documents in the Electronic Health Record during Emergency Care

Summary: Emergency Departments (EDs) seldom conduct Advanced Care Planning (ACP) during patient visits, possibly due to the difficulty in locating ACP documents in the electronic health record (EHR). A study found that ACP documents are lacking for ED patients, even under optimal conditions with social work availability. The multiple potential locations of ACP documents and inaccurate linkage to the EHR are identified as barriers to ACP conversations.

Clinician Communication in Hospice: Constructions of Reality Throughout the End-of-Life Process

The findings revealed three themes, including language choices, roles and responsibilities, and socio-cultural considerations. The Opportunity Model for Presence during the End-of-Life Process (OMP-EOLP) was used to make sense of the findings and discuss implications for language use. The study emphasizes the importance of recognizing the value of presence checks, re-constructing advance care planning, and utilizing different forms of media for timely engagement of end-of-life conversations.

Differences in goals of care discussion outcomes among healthcare professionals: an observational cross-sectional study

Summary: The study revealed significant differences in outcomes between goals of care discussions led by nurses and physicians. Patients were more likely to accept resuscitative measures in nurse-led discussions. Comorbidity burden and illness severity also played a role in patients' decisions regarding cardiopulmonary resuscitation acceptance. Further research is needed to explore contributing factors and improve delivery of goals of care discussions.

A Scoping Review of End-of-Life Communication in International Palliative Care Guidelines for Acute Care Settings

Summary: The scoping review identified 13 guidelines from nine different countries that address end-of-life communication in acute care settings. Thematic analysis revealed eight main themes including purpose and process of communication, cognitive understanding and language, legal aspects, conflicts and barriers, medical record documentation, healthcare professionals' responsibilities and collaboration, education and training, and policies and tools. Differences across countries in guideline development, communication aspects, and emphasis may be influenced by roles of physicians and nurses as well as national laws and regulations.
