Risk Factors for Postoperative Pulmonary Complications Leading to Increased In-Hospital Mortality in Patients Undergoing Thoracotomy for Primary Lung Cancer Resection: A Multicentre Retrospective Cohort Study of the German Thorax Registry

Summary: Postoperative pulmonary complications (PPCs) are common after lung surgery and increase postoperative mortality. This study investigated the incidence of PPCs, in-hospital mortality rate, and risk factors in patients undergoing open thoracotomy lung resections (OTLRs) for primary lung cancer. Male gender, age > 60 years, and smoking were identified as risk factors for PPCs. Other factors such as surgery duration, intraoperative blood loss, and postoperative ventilation were also associated with PPCs. Continuous epidural or paravertebral analgesia was found to reduce the incidence of PPCs.

The effectiveness of ARISCAT Risk Index, other scoring systems, and parameters in predicting pulmonary complications after thoracic surgery

Summary: The effectiveness of the ARISCAT risk index, ASA, preoperative albumin level, and other parameters in predicting postoperative pulmonary complications (PPCs) after thoracic surgery was evaluated. The study found that the ARISCAT risk index and ASA were significant in predicting PPCs and when combined with preoperative albumin levels, they were even more valuable. Age, gender, duration of surgery, and duration of one-lung ventilation (OLV) were also found to be associated with PPCs.

Summary: The study conducted on pulmonary resection patients in Brazil revealed that risk factors for postoperative pulmonary complications include ASA physical status, predicted diffusion capacity of the lungs for carbon monoxide, age, and duration of surgical procedure. These factors were also associated with a prolonged hospital stay.
