Periodontal Management in Periodontally Healthy Orthodontic Patients with Fixed Appliances: An Umbrella Review of Self-Care Instructions and Evidence-Based Recommendations

Federica Di Spirito et al.

Summary: The present study aimed to summarize and evaluate the self-care instructions for orthodontic patients with fixed appliances. Through analyzing 17 systematic reviews, it was found that powered toothbrushes showed short-term improvements in inflammation, bleeding, and periodontal pocket depth values. Chlorhexidine mouthwashes were effective in controlling biofilm accumulation and gingival inflammation. Organic products like aloe vera and chamomile demonstrated antimicrobial properties, and herbal-based mouthwashes showed similar efficacy to chlorhexidine. Motivational methods were also effective in biofilm control and inflammation, while no evidence supported probiotics administration.

Summary: Periodontitis and peri-implantitis are diseases that lead to tooth and dental implant loss, and probiotics have been investigated for their potential beneficial effects. However, there is no definitive conclusion on the effectiveness of probiotics in periodontal management. Further studies are needed to validate their use and provide recommended protocols.

Effects of Multibracket Orthodontic Treatment versus Clear Aligners on Periodontal Health: An Integrative Review

Aaron Jacob David Partouche et al.

Summary: This integrative review found that clear aligners led to better periodontal health compared to multibracket orthodontic appliances. However, more studies are needed to confirm if clear aligners are associated with better gingival conditions. Additionally, variables such as oral hygiene instructions, patient motivation, and supportive treatment may play a more significant role in periodontal evaluation than the type of appliance itself.

Summary: Periodontitis is a common disorder that affects a large percentage of adults globally, with a multifactorial etiology and potential for tooth loss and reduced quality of life. Successful management involves careful diagnosis, treatment including non-surgical and surgical therapies, and long-term maintenance to prevent disease progression and tooth loss.

Prioritization of predisposing factors of gingival hyperplasia during orthodontic treatment: the role of amount of biofilm

Severine Vincent-Bugnas et al.

Summary: The study revealed that approximately 49.7% of patients undergoing fixed orthodontic treatment experienced gingival overgrowth, which was associated with factors such as metallic brackets, mouth breathing, male gender, thick periodontal phenotype, and elastomeric ligations. Logistic regression analysis showed that metallic brackets and treatment duration were the main predictors of gingival enlargement, while the amount of plaque was not directly related to its development during orthodontic treatment.

The Oral Microbiota Changes in Orthodontic Patients and Effects on Oral Health: An Overview

Maria Contaldo et al.

Summary: This study aims to explore the impact of orthodontic treatment on the oral microbiota, potentially leading to the occurrence of caries, periodontal diseases, and other infections that affect the oral and systemic health of orthodontic patients. Comparison with subjects without orthodontic appliances revealed significant differences in supra- and subgingival plaque among orthodontic patients throughout the treatment period, with certain fixed appliance components posing high risks of periodontal disease and tooth decay. Further investigation is needed on the prevalence of Candida spp. and the lack of studies on viruses and protozoas in the oral microbiota of orthodontic patients.

However, among hypertensive ARMD subjects, Clinical Attachment Level values were significantly higher, suggesting a potential link between periodontal disease and ARMD in this subgroup.
