Chronic primary musculoskeletal pain: a new concept of nonstructural regional pain

Previously categorized as myofascial pain syndromes or musculoskeletal pain with a neuropathic component, treatments were anatomically focused. Chronic primary musculoskeletal pain is now recognized as part of chronic primary pain, with the mechanistic descriptor of nociplastic pain. Many patients previously diagnosed with myofascial pain may actually have chronic primary musculoskeletal pain, requiring a paradigm shift in treatment strategies. However, validation of examination techniques, prevalence, ideal treatment, and acceptance by the healthcare community are still needed.
