Summary: This study aimed to assess the clinical and hemodynamic effects of intra-aortic balloon pump (IABP) and Impella devices on patients supported with venoarterial (VA) extracorporeal membrane oxygenation (ECMO). The results showed that ECMO with IABP patients had better survival at 180 days compared to ECMO alone, while the survival in ECMO with Impella patients was not different. ECMO with Impella patients had a higher incidence of bleeding events compared to ECMO alone or ECMO with IABP.

Summary: This study evaluates the association between timing of active left ventricular (LV) unloading and implantation of VA-ECMO with outcomes in patients with cardiogenic shock (CS). The results show that early active LV unloading is associated with a lower risk of 30-day mortality and a higher likelihood of successful weaning from ventilation, without increasing complications.

Benefit of veno-arterial extracorporeal membrane oxygenation combined with Impella (ECpella) therapy in acute coronary syndrome with cardiogenic shock

Summary: The efficacy of ECMO-IABP and ECpella for add-on circulatory support in ACS-CS patients was compared. The results showed that the ECpella group had lower mortality rates, but higher rates of bleeding and new hemodialysis. Among resuscitated patients, the ECpella group had lower mortality rates. The use of ECpella was strongly associated with survival rates.

Left ventricular unloading during extracorporeal life support for myocardial infarction with cardiogenic shock: surgical venting versus Impella device

Summary: The study found that there was no significant difference in 30-day mortality between Impella device and surgical left ventricular vent in patients with cardiogenic shock due to acute myocardial infarction. However, there were significant differences in rates of myocardial recovery and durable mechanical circulatory support. Complication rates were similar between the two groups.
