Coronary flow reserve and cardiovascular outcomes: a systematic review and meta-analysis

Mihir A. Kelshiker et al.

Summary: This meta-analysis aims to quantify the association between reduced coronary flow and all-cause mortality and major adverse cardiovascular events (MACE). The results show that abnormal coronary flow reserve (CFR) is associated with higher incidence of all-cause mortality and MACE. Each 0.1 unit reduction in CFR is associated with a proportional increase in mortality and MACE.

Pericoronary Fat Attenuation Index Is Associated With Vulnerable Plaque Components and Local Immune-Inflammatory Activation in Patients With Non-ST Elevation Acute Coronary Syndrome

Jia Teng Sun et al.

Summary: This study assessed the relationship between pericoronary FAI values and plaque components and cytokine levels in patients with non-ST elevation acute coronary syndrome. The study found that higher pericoronary FAI values were associated with a higher prevalence of calcification and low-attenuation plaques. Additionally, increased necrotic core volume and fibrofatty volume were positively associated with pericoronary FAI values, while fibrous volume showed a negative association. Furthermore, lesions with higher pericoronary FAI values exhibited an increasing proinflammatory intracellular cytokine profile.

Coronary Physiology: Delivering Precision Medicine?

Laura Maitre-Ballesteros et al.

Summary: Coronary physiological assessment is widely used to evaluate coronary lesions. Fractional flow reserve (FFR) is the gold standard method for selecting revascularization. In addition to FFR, other indexes are available for exploring the coronary microcirculation. Coronary physiological assessment offers the potential of precision medicine for patients with coronary artery disease.

Summary: This study aimed to evaluate the agreement between FFR and iFR in patients with intermediate LMCA stenosis and to validate the safety of deferring revascularization based on a hybrid decision-making strategy combining iFR and IVUS. The results showed moderate concordance between FFR and iFR, with IVUS being useful in guiding revascularization in cases of discordance. Deferral of revascularization based on iFR appeared to be safe in terms of major adverse cardiac events.
