Prognosis and subtype analysis of left ventricular noncompaction in adults: A retrospective multicenter study

Yunfei Feng et al.

Summary: This study aimed to investigate the prognosis of left ventricular noncompaction (LVNC) and the distribution of different subtypes, and compare their prognosis. The results showed that age, LVEF<50%, and ventricular tachycardia/ventricular fibrillation were significantly associated with the risk of adverse cardiovascular events in LVNC patients. The most common subtype was dilated LVNC, which had a worse prognosis.

Long-Term Effects of Sacubitril-Valsartan on Cardiac Remodeling: A Parallel Echocardiographic Study of Left and Right Heart Adaptive Response

Federica Valli et al.

Summary: Sacubitril/Valsartan (S/V) has potential anti-remodeling effects and improves right ventricular (RV) coupling to the pulmonary circulation. This study found that 27.6% of HFrEF patients experienced left ventricular (LV) reverse remodeling and there was a significant improvement in RV function. These findings suggest a potential long-term reverse global remodeling effect of S/V and highlight a possible direct effect of the drug on the interaction between RV and pulmonary circulation.

A Rare Case of Left Ventricular Non-Compaction with Coronary Artery Anomaly Complicated by ST-Elevation Myocardial Infarction and Subcutaneous Defibrillator Implantation

Francesca Romana Prandi et al.

Summary: Left ventricular non-compaction (LVNC) is a rare congenital cardiomyopathy characterized by ventricular hypertrabeculation. This case report describes a 33-year-old male with a family history of sudden cardiac death (SCD) who presented with chest pain and was diagnosed with anterior ST-elevation myocardial infarction (STEMI). Further evaluation revealed an anomalous coronary artery origin and confirmed the diagnosis of LVNC. Due to the severe ventricular dysfunction and family history of SCD, the patient underwent subcutaneous ICD implantation for primary prevention.

Multimodality Cardiac Imaging in Cardiomyopathies: From Diagnosis to Prognosis

Guillem Casas et al.

Summary: Cardiomyopathies encompass various types of myocardial disorders, and the diagnosis and prognosis assessment require multimodality cardiac imaging techniques. Transthoracic echocardiography is the preferred modality, while cardiovascular magnetic resonance provides more accurate information. Regular cardiac imaging testing is necessary for patients with cardiomyopathies due to the evolving natural history of the disease.

Acute coronary syndromes in diabetic patients, outcome, revascularization, and antithrombotic therapy

Elena Emilia Babes et al.

Summary: Diabetes worsens the progression of atherosclerosis, increasing the risk of developing acute coronary syndrome (ACS). Diabetic patients experience earlier onset ACS and have poorer outcomes due to increased inflammation and thrombosis. Despite advancements in treatment, diabetic patients still have worse outcomes compared to non-diabetic patients.

Summary: This study investigated the impact of mitral regurgitation (MR) on cardiovascular magnetic resonance characteristics and clinical outcomes in patients with left ventricular noncompaction (LVNC). The results demonstrated that MR induced more maladaptive left ventricular remodeling and the incidence of adverse outcomes may be related to the degree of MR.

Summary: The prognosis of LVNC and DCM patients is similar, but LVNC patients have higher left ventricular ejection fractions (LVEF). Among patients with LVEF <= 45%, the LVNC group had a higher occurrence of the primary endpoint, especially hospitalization for heart failure and heart transplantation.

Molecular mechanisms of sacubitril/valsartan in cardiac remodeling

Nor Hidayah Mustafa et al.

Summary: Sacubitril/valsartan is a pharmaceutical strategy that increases natriuretic peptide levels and shows significant improvement in heart failure patients. It reduces morbidity and mortality through mechanisms like reverse cardiac remodeling and has benefits even for patients with preserved ejection fraction.

Prognostic Value of Late Gadolinium Enhancement in Left Ventricular Noncompaction: A Multicenter Study

Wei Huang et al.

Summary: In this study, the clinical and cardiovascular magnetic resonance data of 75 patients with left ventricular noncompaction were retrospectively analyzed. The results showed a correlation between late gadolinium enhancement types and major adverse cardiovascular events. A specific ring-like pattern and involvement of the free wall or mid-wall were associated with an increased risk of adverse cardiovascular events. The extent of late gadolinium enhancement greater than 7.5% and the presence of ring-like, multiple segments, and free-wall enhancement were significantly associated with major adverse cardiovascular events.

Summary: Left ventricular non-compaction (LVNC) is a highly heterogeneous disorder with variable clinical presentation, morphologic appearance, and prognosis. It is still unclear if LVNC should be classified as a separate cardiomyopathy and if the hypertrabeculated phenotype is reversible. Current diagnostic criteria have limitations.

Left ventricular noncompaction-a rare cause of triad: heart failure, ventricular arrhythmias, and systemic embolic events: a case report

Despina Toader et al.

Summary: Left ventricular noncompaction is a rare cardiomyopathy characterized by a thin, compacted epicardial layer and a noncompacted endocardial layer, with trabeculations and recesses communicating with the left ventricular cavity. In advanced stages, heart failure, ventricular arrhythmia, and systemic embolization are common symptoms. The apex and mid inferior and lateral walls of the left ventricle are primarily affected, with possible involvement of the right ventricular apex as well.

Zhihua Pang et al.

Summary: The sequential treatment with rhBNP followed by sacubitril/valsartan in acute heart failure patients showed better outcomes compared to standard treatment and rhBNP alone. This may be linked to decreased levels of inflammatory factors and correction of oxidative stress imbalance, resulting in improved cardiac structure and pulmonary artery pressure. Sacubitril/valsartan also significantly reduced serum levels of inflammatory factors while modulating oxidizing and antioxidant factors.
