Summary: This review highlights the significant risk factors for recurrence in low-risk endometrial carcinoma, including CTNNB1 mutation, L1CAM expression, lymphovascular invasion, and tumor size. Some low-risk EC patients are at high risk of recurrence, and suspicion should be heightened when they have positive molecular markers, large tumors, and lymphovascular invasion. There is a need for a novel scoring system and randomized controlled trials to identify patients who will benefit most from adjuvant therapy to prevent recurrence.
