Invasive Fungal Breakthrough Infections under Targeted Echinocandin Prophylaxis in High-Risk Liver Transplant Recipients

Summary: Invasive fungal infections are common complications after liver transplantation, and the choice of antifungal agent for prophylaxis is still a topic of debate. Recent data suggest concerns about the efficacy of echinocandins in preventing breakthrough infections, particularly in cases of intra-abdominal candidiasis. A retrospective study on high-risk liver transplant recipients receiving echinocandin prophylaxis showed a breakthrough incidence of 16%, which had significant effects on postoperative complications, graft survival, and mortality. The reasons for the breakthrough infections may be multifactorial, including the emergence of Candida parapsilosis and the development of echinocandin resistance in Candida glabrata. Further studies are needed to clarify the efficacy of echinocandin prophylaxis in liver transplantation.

Incidence of Invasive Fungal Infections in Liver Transplant Recipients under Targeted Echinocandin Prophylaxis

Summary: Invasive fungal infections (IFIs) are a significant risk after liver transplantation, and the use of antifungal prophylaxis is common. This study aimed to investigate the incidence of IFIs under targeted echinocandin prophylaxis in high-risk liver transplant recipients. The results showed that despite prophylaxis, there was still a notable risk for IFIs, especially in patients with certain risk factors. It is important to critically evaluate the use of echinocandins for prophylaxis and adhere to internal prophylaxis algorithms to minimize the risk of IFIs.

In-hospital mortality of liver transplantation and risk factors: a single-center experience

This study analyzed in-hospital mortality rates and factors correlated with death after LT.
