Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Christopher J. L. Murray et al.

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

Higher abundance of core antimicrobial resistant genes in effluent from wastewater treatment plants

Shahbaz Raza et al.

Summary: The study identified the persistent core resistome in urban wastewater treatment plants, showing that current technologies are unable to completely remove antimicrobial resistant bacteria and genes. The study highlighted the presence of key antibiotic resistant genes in both influent and effluent samples, especially in opportunistic pathogens such as Acinetobacter baumannii and Pseudomonas aeruginosa. Further research is needed to address the public health concern of the environmental dissemination of antibiotic resistant genes by wastewater treatment plants.

High-Throughput Screening of Antimicrobial Resistance Genes and Their Association With Class 1 Integrons in Urban Rivers in Japan

Ikuro Kasuga et al.

Summary: Antimicrobial resistance (AMR) is a serious public health concern. This study used high-throughput quantitative polymerase chain reaction (HT-qPCR) to characterize the pollution profiles of antimicrobial resistance genes (ARGs) and mobile gene elements (MGEs) in urban rivers in Tokyo and its surrounding area. The abundance of class 1 integron-integrase gene (intI1) and the array of class 1 integron gene cassettes were also determined. The results showed that effluent discharge from wastewater treatment plants increased the abundance of ARGs and MGEs, and intI1 could be used as a proxy for monitoring these genes in urban rivers.

The prevalence and removal of antibiotic resistance genes in full-scale wastewater treatment plants: Bacterial host, influencing factors and correlation with nitrogen metabolic pathway

Qian Ping et al.

Summary: This study investigated the prevalence of antibiotic resistance genes (ARGs) in wastewater treatment plants (WWTPs) and explored the correlation between various influencing factors and ARGs. The Unitank process was found to be inefficient in removing typical ARGs compared to other processes. Antibiotics, flow rate, and nutrients were identified as influential factors affecting the occurrence of ARGs in wastewater. Factors such as integrons, wastewater characteristics, antibiotics, and metals comprehensively affected the distribution of ARGs in waste activated sludge (WAS). Dissimilatory nitrate reduction played a significant role in the fate of ARGs during wastewater treatment processes. This study provides new insights into the occurrence and dissemination of ARGs in WWTPs.

Metagenomic profiling and transfer dynamics of antibiotic resistance determinants in a full-scale granular sludge wastewater treatment plant

David Calderon-Franco et al.

Summary: In the context of One Health, wastewater treatment plants play a crucial role in preventing the dissemination of antimicrobial resistance. However, current surveillance studies focus mainly on monitoring intracellular DNA, while the role of extracellular free DNA is often overlooked. This study found that in the process of wastewater treatment, the exDNA fractions containing resistance determinants and mobile genetic elements need to be considered for AMR surveillance and risk assessment.

Profiling of emerging pathogens, antibiotic resistance genes and mobile genetic elements in different biological wastewater treatment plants

Thobela Conco et al.

Summary: Insufficiently treated effluents from wastewater treatment systems can lead to the emergence and spread of bacterial pathogens, antibiotic resistant genes (ARGs), and mobile genetic elements (MGEs), posing a risk to the environment and human health.

Initial behaviors and removal of extracellular plasmid gene in membrane bioreactor

Rongxuan Wang et al.

Summary: Extracellular antibiotic resistance genes (eARG) play a crucial role in the spread of antimicrobial resistance (AMR) in wastewater treatment and aquatic environments. Membrane bioreactors (MBRs) are more effective at removing ARGs from wastewater compared to traditional activated sludge processes. However, the removal of eARGs may be limited by their size, preventing them from passing through microfiltration (MF) membranes.

Change in clinical practice variations for antibiotic prescriptions across different pediatric clinics: A Japan's nationwide observational study

Yusuke Okubo et al.

Summary: This study examined the changing trends of antibiotic prescription patterns in different clinics in Japan from 2013 to 2018, and found varied impacts of the National Action Plan on different clinics. One quarter of the clinics did not improve antibiotic prescription patterns even after the introduction of the Action Plan.

Ayumi Shishido et al.

Summary: Outpatient medical facilities, particularly pediatric primary emergency care centers in Japan, face challenges in implementing conventional antimicrobial stewardship programs. A nudge-based ASP utilizing newsletters was found to significantly reduce unnecessary 3GC prescriptions in a Japanese PEC, showing potential for wider applications in outpatient settings.
