Fascial defect closure versus bridged repair in laparoscopic ventral hernia mesh repair: a systematic review and meta-analysis of randomized controlled trials

Summary: This study systematically reviewed and meta-analyzed randomized controlled trials comparing primary fascial defect closure (FDC) versus bridged repair during laparoscopic ventral hernia mesh repair. The results indicate that there is no significant difference between the two techniques in terms of recurrence risk, seroma formation risk, risks of clinical or radiological eventration, and chronic pain. However, the evidence on how both techniques affect postoperative quality of life (QoL) or early postoperative pain is conflicting. Further research with narrow inclusion criteria for hernia size focusing on both primary and secondary hernias is recommended.

Summary: This study compared the clinical outcomes of open versus laparoscopic parastomal hernia repair and found that laparoscopic repair was associated with shorter operative duration, decreased length of stay, fewer short-term wound complications, and increased longevity of repair. Additional research is needed to further investigate methods associated with fewer complications and increased durability in parastomal hernia repair.

After correcting for width, most outcomes do not significantly differ between the two types, except for medical complications being more common after incisional hernia repair.

Summary: In comparing robotic-assisted ventral hernia repair (rVHR) to traditional endo-laparoscopic ventral hernia repair (lapVHR), it was found that lapVHR had shorter operative times but higher rates of open conversion. Patient-reported outcomes did not show significant differences between the two methods. Proper patient selection is important to maximize the benefits of rVHR.
