Keep it Simple, Laboratory Parameters to Predict Complicated Acute Appendicitis in Children Younger Than Five Years

Danny Hazan et al.

Summary: This study aimed to evaluate the role of routine blood tests in differentiating between complicated and uncomplicated acute appendicitis. The retrospective analysis showed that C-reactive protein, neutrophil/lymphocyte ratio, and platelet/lymphocyte ratio could be used as simple laboratory parameters to identify complicated appendicitis in children aged 5 years or younger. These universal parameters may guide the treatment and surgical decision-making for a difficult-to-diagnose population.

Using Microbiology Culture in Pediatric Appendicitis to Risk Stratify Patients: A Cohort Study

Benjamin Martin et al.

Summary: This study investigates the value of intra-operative microbiology samples in pediatric appendicitis. The results show that obtaining microbiology samples during surgery is associated with post-operative complications. The proposed benefits include tailoring antimicrobial management, risk-stratifying patients, and reducing intra-abdominal abscess formation.

Microbiota Assessment of Pediatric Simple and Complex Acute Appendicitis

Mohit Kakar et al.

Summary: This study identified the prevalent microbiota in pediatric patients with acute appendicitis and evaluated the antibacterial sensitivity of the isolated microorganisms. E. coli was found to be the main causative agent, susceptible to various antibiotics. P. aeruginosa was more common in cases of acute complex appendicitis.

Summary: Escherichia coli and Bacteroides species are the dominant bacteria in both perforated and non-perforated appendicitis, but Pseudomonas aeruginosa, Streptococcus anginosus group, and Enterococcus group are more prevalent in perforated appendicitis.

Impact of the COVID-19 pandemic on appendicitis treatment in Germany-a population-based analysis

Franziska Kohler et al.

Summary: During the COVID-19 pandemic in early 2020, there was a noticeable decrease in patients presenting with acute appendicitis, particularly among females < 40 years old. The number of patients with complicated appendicitis remained unchanged, while treatment and complication rates for acute appendicitis did not differ.

Is Pseudomonas infection associated with worse outcomes in pediatric perforated appendicitis?

Christina M. Theodorou et al.

Summary: This study found that 22% of children with perforated appendicitis had Pseudomonas infection, which was associated with longer antibiotic durations and length of stay, but similar rates of postoperative infections, interventions, and readmissions compared to children without Pseudomonas. Empiric coverage of Pseudomonas may not be necessary.

Summary: This study found an increased proportion of Gram-positive cocci and Milleri group Streptococci in the intraperitoneal flora of patients with complicated appendicitis. Patients with Gram-positive cocci had longer hospital stays and more complicated diseases. Those with Milleri group Streptococci isolates had a higher readmission rate.

Appendicitis during the COVID-19 lockdown: results of a multicenter analysis in Germany

Arnulf G. Willms et al.

Summary: The COVID-19 lockdown in Germany had significant impacts on abdominal emergency surgery, leading to a decrease in the number of appendectomies, an increase in the rate of complicated appendicitis, a decrease in the absolute number of complicated appendicitis, and a significant reduction in the rate of negative appendectomies. However, overall postoperative morbidity and mortality remained unchanged.

Changes in appendicitis treatment during the COVID-19 pandemic - A systematic review and meta-analysis

Franziska Koehler et al.

Summary: During the COVID-19 pandemic, there was a decrease in adult appendicitis cases and an increase in pediatric cases. The rate of open appendectomies showed a slight increase while antibiotic treatment rate significantly rose. Adults had higher rates of complicated appendicitis, and there were no significant changes in time to consultation and length of hospital stay.

Appendectomy Is Associated With Alteration of Human Gut Bacterial and Fungal Communities

Shuntian Cai et al.

Summary: Recent research has shown the significant impact of appendectomy on gut bacteria and fungi, with a particular enduring effect on fungi composition and diversity. Samples from individuals with a history of appendectomy displayed less diverse bacterial composition and more complex fungal-bacterial community interactions compared to samples from those without appendectomy. The study suggests a trend toward restoration of the normal bacterial microbiome over time after appendectomy.

Management and Microbiology of Perforated Appendicitis in Pediatric Patients: A 5-Year Retrospective Study

Alex S. Plattner et al.

Summary: The study found that Bacteroides fragilis and E. coli were the most common intra-abdominal microbes in children with perforated appendicitis. The routine use of broad-spectrum antibiotics or continuation of antibiotics beyond discharge was not correlated with improved clinical outcomes. White blood cell counts were not correlated with meaningful clinical outcomes.
