Present status and future directions - irrigants and irrigation methods

Summary: Irrigation is the primary method for cleaning and disinfection of the root canal system, but it faces challenges in eliminating bacteria located in the root canal system's intricate structures. Sodium hypochlorite is the preferred irrigant, and syringe and ultrasonic activation are the most popular irrigation methods. There is no evidence that adjunct irrigation methods can improve treatment outcomes beyond what can be achieved by instrumentation and syringe irrigation. Future research should focus on redefining priorities, investigating the penetration and effects of irrigants, and conducting clinically relevant comparisons.

Comparative evaluation of intermediate solutions in prevention of brown precipitate formed from sodium hypochlorite and chlorhexidine gluconate

Summary: This study evaluated the efficacy of intermediate treatments in preventing the formation of toxic brown precipitate during root canal therapy. The results showed that using 5% sodium thiosulfate significantly reduced the intensity and area of the brown precipitate. Analysis using ToF-SIMS revealed that 5% sodium thiosulfate was most effective in reducing the components representing para-chloroaniline and chlorhexidine gluconate.

Assessment of the cytotoxic effects and chemical composition of the insoluble precipitate formed from sodium hypochlorite and chlorhexidine gluconate

Summary: The study aimed to investigate the cytotoxic potential of the brown precipitate formed with NaOCl and CHX using a small animal model and human gingival fibroblasts, and to analyze the chemical composition of the precipitate using ToF-SIMS. The results showed that the brown precipitate was toxic to both C. elegans larvae and hTERT-hNOF cells, with ToF-SIMS analysis revealing ions characteristic of CHX and the carcinogen PCA, which may explain the observed toxicities. Consecutive use of CHX and NaOCl irrigants in root canal treatment should be avoided due to the insoluble and toxic nature of the brown precipitate.
