Axial Spondyloarthritis and Diagnostic Challenges: Over-diagnosis, Misdiagnosis, and Under-diagnosis

Summary: This article reviews the challenges in diagnosing axial spondyloarthritis and identifies possible contributing factors. Recent findings suggest that the lack of accurate and timely diagnosis can lead to treatment delays and worse disease outcomes. The misuse of diagnostic criteria and inter-reader variability in interpreting images are also contributing factors. Further investigation is needed to understand the diagnostic contribution of inflammatory back pain features, serologic biomarkers, and genetics to axial spondyloarthritis.

Diagnostic delay in axial spondyloarthritis: a systematic review and meta-analysis
