Summary: Bacterial vaginosis is the most common cause of vaginal discharge and has a high recurrence rate. The biofilm formed by bacterial communities in the vagina may contribute to the persistence of the disease after treatment, along with independent bacterial properties that confer antimicrobial resistance. Current treatment recommendations for refractory and recurrent bacterial vaginosis involve combination therapy and alternative approaches.

Mixed Vaginitis Due to Bacterial Vaginosis and Candidiasis

Summary: This study reviewed clinical cases of mixed vaginitis and summarized their demographics, clinical presentation, diagnosis, and treatment. The diagnosis of mixed vaginitis was mainly based on clinical suspicion and point of care diagnostic tests. Most patients had a history of long-standing recurrent vaginitis. Dual antimicrobial therapy achieved short term therapeutic success, but some Candida strains showed resistance to fluconazole.

Summary: The study conducted in a large urban public hospital in Melbourne examined the prevalence of Trichomonas vaginalis (TV) infection among STI patients, finding a prevalence rate of 1.7%, higher than Neisseria gonorrhoeae but lower than Chlamydia trachomatis, with the highest rate in females aged 30-44 years. The use of multiplex polymerase chain reaction (MPCR) testing significantly improved TV detection compared to clinician request-based testing, indicating potential for better screening strategies in urban settings. Current targeted testing guidelines for TV were found to be inadequate in identifying cases in urban areas.

Effective management and laboratory diagnosis are crucial to prevent inappropriate therapy and recurrence of mixed vaginitis.

Recurrent Bacterial Vaginosis: An Unmet Therapeutic Challenge. Experience With a Combination Pharmacotherapy Long-Term Suppressive Regimen

Summary: This study suggests that a combination therapy of oral nitroimidazole and vaginal boric acid followed by maintenance metronidazole gel can effectively reduce symptomatic recurrence of bacterial vaginosis in patients with RBV.
