Guidelines for Genetic Testing and Management of Alport Syndrome

Judy Savige et al.

Summary: Genetic testing for pathogenic COL4A3?5 variants is crucial for investigating persistent hematuria and suspected inherited kidney diseases. Even in cases where a heterozygous pathogenic COL4A3 or COL4A4 is suspected, experts recommend genetic testing and cascade testing of family members to reduce the risk of kidney dysfunction. Testing for variants in the COL4A3?5 genes is also recommended for other kidney conditions such as proteinuria, steroid-resistant nephrotic syndrome, IgA glomerulonephritis, and kidney failure of unknown cause.

Genotype-phenotype correlations for COL4A3-COL4A5 variants resulting in Gly substitutions in Alport syndrome

Joel T. Gibson et al.

Summary: This study examined the molecular characteristics of Gly substitutions in Alport syndrome and found that the stability of the substitution and its interaction with nearby residues determine the risk of haematuria, early onset kidney failure, and hearing loss in this inherited kidney disease.

Genotype-Phenotype Correlations for Pathogenic COL4A3-COL4A5 Variants in X-Linked, Autosomal Recessive, and Autosomal Dominant Alport Syndrome

Judy Savige et al.

Summary: Alport syndrome is a genetic disease caused by pathogenic variants in COL4A3 - COL4A5 genes, leading to kidney failure and hearing loss. Understanding the genotype-phenotype correlations can help predict the age of kidney failure and guide clinical management.

Mild X-linked Alport syndrome due to the COL4A5 G624D variant originating in the Middle Ages is predominant in Central/East Europe and causes kidney failure in midlife

Aleksandra M. Zurowska et al.

Summary: In a study of children with X-linked Alport Syndrome in Poland, a single variant in COL4A5 was identified as the predominant cause, dating back to a founder effect in the 12-13th century. This variant, known as G624D, was associated with a milder clinical course compared to other pathogenic variants, but still conferred a significant risk for kidney failure and deafness. Males with the G624D variant are candidates for both existing and emerging therapies for Alport Syndrome.

Clinical and Genetic Features of Autosomal Dominant Alport Syndrome: A Cohort Study

Monica Furlano et al.

Summary: The study evaluated the clinical and genetic spectrum of patients with autosomal dominant Alport syndrome (ADAS) in 82 families. It found that ADAS patients have a wide range of clinical presentations, with a median kidney survival of 67 years and microhematuria being the most common kidney manifestation.

Prevalence Estimates of Predicted Pathogenic COL4A3-COL4A5 Variants in a Population Sequencing Database and Their Implications for Alport Syndrome

Joel Gibson et al.

Summary: This study estimated the frequencies of predicted pathogenic COL4A3-COL4A5 variants in populations without known kidney disease. The COL4A5 variants were found in at least one in 2320 individuals, and the predicted pathogenic heterozygous COL4A3 and COL4A4 variants affected one in 106 of the population. The results suggest the population frequencies for Alport syndrome need to be adjusted for individual variant penetrance and other factors.
