Summary: This study evaluated the efficacy of narrow-band ultraviolet B (NB-UVB) phototherapy, methotrexate, and combined NB-UVB phototherapy with methotrexate in the treatment of psoriasis vulgaris and their effects on serum cathelicidin and vitamin D. The results showed that the combined NB-UVB phototherapy with methotrexate had the highest clinical improvement of psoriasis vulgaris, with an increase in serum vitamin D and a decrease in cathelicidin.

Total 25(OH)D is a reliable measure for vitamin D status in psoriasis patients, but evaluation of free 25(OH)D levels should be considered in patients with multiple co-morbidities and/or undergoing biologic treatment.

Vitamin D Signaling in Psoriasis: Pathogenesis and Therapy

Summary: Psoriasis is a systemic, chronic, immune-mediated disease whose etiology and pathogenesis remain unknown. T-cell activation plays a key role in the development of psoriasis. Modulation of the local neuroendocrine system and vitamin D signaling have shown promising results in adjuvant treatment for psoriasis. Phototherapy, especially UVB-based, can affect serum levels of 25(OH)D, but more clinical trials are needed to establish the correlation between these changes and improvement in psoriasis.

Impact of Etanercept on Vitamin D Status and Vitamin D-binding Protein in Bio-naive Patients with Psoriasis

Summary: Higher levels of serum vitamin D-binding protein have been found in patients with psoriasis compared to healthy controls, suggesting a potential role in inflammation. Treatment with etanercept for 24 weeks resulted in improvements in psoriasis and a decrease in highly sensitive C-reactive protein levels. Higher baseline levels of vitamin D were associated with faster and greater improvement in psoriasis, indicating a potential predictive value for treatment outcomes.
