Cost-effectiveness of domiciliary non-invasive ventilation in patients with chronic obstructive pulmonary disease

James Hall et al.

Summary: Domiciliary NIV is likely cost-effective for posthospitalised patients, but unlikely to be cost-effective in stable populations. ICERs were close to the £20,000/QALY threshold for stable Global Initiative for Chronic Obstructive Lung Disease (GOLD) stage 4 patients or those with higher mortality and exacerbation risks.

Home mechanical ventilation: the Dutch approach

R. J. M. van den Biggelaar et al.

Summary: This paper provides information about the evolution of Home Mechanical Ventilation (HMV) in the Netherlands over the past 30 years, including details about the number of patients, different diagnostic groups, residence, and types of ventilators. In the Netherlands, there are four centers responsible for all patients in need of HMV, with nationwide criteria for referral and initiation. A unique national learning management system has also been developed for caregivers and professionals.

Predictors of mortality in chronic obstructive pulmonary disease: a systematic review and meta-analysis

Catherine Owusuaa et al.

Summary: This study aimed to identify predictors of mortality in patients with chronic obstructive pulmonary disease (COPD) in order to facilitate advance care planning discussions. The study found that previous hospitalization for acute exacerbation, hospital readmission within 30 days, cardiovascular comorbidity, age, male sex, and long-term oxygen therapy were significant predictors of mortality. The existing multicomponent prognostic models showed moderate discriminative ability. Further improvement in the discriminative ability or development of new prognostic models is needed for large-scale implementation.

Takamasa Kitajima et al.

Summary: This study aimed to investigate the relationship between episodic nocturnal hypercapnia (eNH) and COPD exacerbation. The results showed that eNH may be associated with a history of more frequent exacerbations in COPD patients with a daytime arterial carbon dioxide pressure (PaCO2) < 55.0 mmHg.

Long-Term Domiciliary Noninvasive Ventilation for COPD

Robert L. Owens

Summary: COPD can cause abnormalities in oxygenation and ventilation. Supplemental oxygen therapy has been proven effective, while efforts to increase ventilation have been less successful. Recent studies show that high-intensity noninvasive ventilation can improve quality of life and reduce mortality, but identifying and treating COPD patients who could benefit from this treatment still presents challenges.

Marius Lebret et al.

Summary: This study compared the effect of nasal and oronasal masks on NIV treatment efficacy and adherence in patients with COPD and OHS. The results showed that oronasal masks are most commonly used for home NIV delivery in these patients, but there is no difference in efficacy or tolerance between oronasal and nasal masks.

Intrinsic Dynamic Positive End-Expiratory Pressure in Stable Patients with Chronic Obstructive Pulmonary Disease

Michele Vitacca et al.

Summary: The study found that in stable COPD patients, hypercapnic patients had significantly higher intrinsic dynamic positive end-expiratory pressure (PEEPi,dyn) compared to normocapnic patients. There were weak correlations between PEEPi,dyn and various noninvasive measurements of lung and respiratory muscle function. Transdiaphragmatic pressure was identified as the strongest variable associated with PEEPi,dyn.
