CD16(+) monocytes expressed the highest IFN gene score and showed an inflammatory and antigen-presenting phenotype. A distinct transitional B cell population was also found in JDM.

Summary: This study identifies key immune cell types and signaling pathways involved in the pathogenesis of anti-melanoma differentiation-associated gene 5-positive dermatomyositis (MDA5(+) DM). The findings highlight the importance of type I interferons in MDA5(+) DM pathology and suggest potential future tailored therapies based on these immune-pathogenic features. High frequency of circulating ISG15(+) CD8(+) T cells at baseline predicts poor one-year survival in MDA5(+) DM patients, emphasizing the clinical relevance of the immune signatures identified in this study.

Current knowledge on the pathogenic mechanisms of the disease is still in its infancy, but evidence supports a central role of interferon-mediated vasculopathy in lesion development.
