Antimicrobial susceptibility of commensal Neisseria in a general population and men who have sex with men in Belgium

Jolein Gyonne Elise Laumen et al.

Summary: Non-pathogenic Neisseria are a reservoir of antimicrobial resistance genes for pathogenic Neisseria meningitidis and Neisseria gonorrhoeae. Men who have sex with men (MSM) are at risk of co-colonization with resistant non-pathogenic and pathogenic Neisseria. Our study found that non-pathogenic Neisseria among MSM show lower susceptibility to azithromycin and ciprofloxacin compared to a general population, while susceptibility to ceftriaxone did not differ significantly. Antimicrobial exposure did not have a significant impact on susceptibility of non-pathogenic Neisseria in MSM.

Awareness of, Willingness to Take PrEP and Its Actual Use Among Belgian MSM at High Risk of HIV Infection: Secondary Analysis of the Belgian European MSM Internet Survey

Veerle Buffel et al.

Summary: The study found that one third of MSM in Belgium were eligible for PrEP, but PrEP awareness was generally low. MSM unwilling to use PrEP were often those with lack of knowledge about PrEP, higher self-efficacy in safe sex, having a steady partner, and reporting more symptoms of depression. Those not actually using PrEP were more likely to live in small cities and experience financial problems.

Markedly Increasing Antibiotic Resistance and Dual Treatment of Neisseria gonorrhoeae Isolates in Guangdong, China, from 2013 to 2020

Xiaomian Lin et al.

Summary: The emergence of multidrug resistance in Neisseria gonorrhoeae, including resistance to azithromycin and extended-spectrum cephalosporins, is a concerning issue. This study confirms the antibiotic resistance trends and suggests that dual therapy could be an effective solution for treating refractory N. gonorrhoeae in Guangdong, China.

Europe-wide expansion and eradication of multidrug-resistant Neisseria gonorrhoeae lineages: a genomic surveillance study

Leonor Sanchez-Buso et al.

Summary: The European gonococcal population in 2018 showed an increase in azithromycin-resistant N. gonorrhoeae and a rise in susceptibility to ceftriaxone and cefixime. The emergence of new lineages associated with antimicrobial resistance highlights the importance of enhanced surveillance programs. Overall, the study emphasizes the need for genomic surveillance combined with epidemiological and AMR data to understand the dynamics of gonorrhoea epidemiology.

Background review for the '2020 European guideline for the diagnosis and treatment of gonorrhoea in adults'

M. Unemo et al.

Summary: Gonorrhoea is a significant public health concern globally, and the 2020 European guideline provides updated evidence-based guidance on diagnosis and treatment to address increasing incidence and concerns about ceftriaxone resistance. The guideline emphasizes the importance of increased testing, dual antimicrobial therapy, test of cure, and enhanced surveillance to control and mitigate the spread of antimicrobial resistance in gonorrhoea.

The Mosaic mtr Locus as Major Genetic Determinant of Azithromycin Resistance of Neisseria gonorrhoeae-Germany, 2018

Sebastian Banhart et al.

Summary: The resistance of Neisseria gonorrhoeae to azithromycin within the German Gonococcal Resistance Network increased from 4.3% in 2016 to 9.2% in 2018. The rise in resistance was primarily clonal and linked to a specific mtr locus. The international spread of resistant lineages was identified as a major factor contributing to azithromycin resistance in Germany.

Antibacterial mouthwash to prevent sexually transmitted infections in men who have sex with men taking HIV pre-exposure prophylaxis (PReGo): a randomised, placebo-controlled, crossover trial

Christophe Van Dijck et al.

Summary: The study aimed to evaluate the use of Listerine mouthwash in reducing bacterial sexually transmitted infections among HIV pre-exposure prophylaxis (PrEP) users. Results showed that Listerine did not significantly reduce the incidence of STIs in this population.

Changes in the prevalence of self-reported sexually transmitted bacterial infections from 2010 and 2017 in two large European samples of men having sex with men-is it time to re-evaluate STI-screening as a control strategy?

Ulrich Marcus et al.

Summary: Between 2010 and 2017, overall diagnoses with gonorrhoea/chlamydia and syphilis increased by 76% and 83% across countries. The increase was more pronounced for asymptomatic infections. The proportion of respondents screened and the frequency of screening grew considerably. Condomless anal intercourse with the last non-steady partner rose by 62%, and self-reported partner numbers grew.

Choosing New Therapies for Gonorrhoea: We Need to Consider the Impact on the Pan-Neisseria Genome. A Viewpoint

Chris Kenyon et al.

Summary: While new gonorrhoea treatment guidelines typically focus on resistance in Neisseria gonorrhoeae, it is important to consider resistance in commensal Neisseria species as well, as resistance patterns can be passed on to N. gonorrhoeae. This highlights the importance of evaluating the impact of gonococcal therapies on resistance in commensal Neisseria.
