Polymer- and lipid-based nanocarriers for ocular drug delivery: Current status and future perspectives

Haijie Han et al.

Summary: Ocular diseases have a significant impact on patients' vision and quality of life, with a global prevalence of over 43 million blindness. However, delivering drugs effectively to the eyes, especially for intraocular disorders, remains a major challenge due to various ocular barriers. Recent advancements in nanocarrier technology provide a promising solution by improving drug penetration, retention, solubility, toxicity reduction, release duration, and targeted drug delivery. This review highlights the progress and applications of nanocarriers, specifically polymer- and lipid-based nanocarriers, in treating different eye diseases and their potential for efficient ocular drug delivery. It also discusses ocular barriers, administration routes, and future prospects and challenges in the field of nanocarriers for ocular disease treatment.

Highly Retina-Permeating and Long-Acting Resveratrol/Metformin Nanotherapeutics for Enhanced Treatment of Macular Degeneration

Duc Dung Nguyen et al.

Summary: We developed a nanomedicine strategy to create nanotherapeutics with high retinal permeability and sustained bioactive delivery. The nanotherapeutics were designed through aminolysis of resveratrol-encapsulated polycaprolactone nanoparticles, followed by the formation of amide linkages with a transacting activator of transcription cell penetrating peptide and metformin. In vitro and in vivo studies demonstrated the effectiveness of the nanotherapeutics in improving retinal permeability and targeting retinal diseases.

Prophylaxis of posterior capsule opacification through autophagy activation with indomethacin-eluting intraocular lens

Xiaobo Zhang et al.

Summary: Posterior capsule opacification (PCO) is a common complication of cataract surgery, but a novel indomethacin-eluting intraocular lens (IOL) with sustained drug release behavior showed excellent anti-inflammatory and anti-PCO effects in a rabbit model. Indomethacin suppressed cell migration and down-regulated COX-2 and EMT markers, as well as promoted autophagy activation in lens epithelial cells (LECs), making it a promising pharmacological intervention for PCO prophylaxis. The developed IOL, with its sustained indomethacin release property, served as a drug delivery platform and prevented PCO in vivo.

Drug-eluting intraocular lens with sustained bromfenac release for conquering posterior capsular opacification

Xiaobo Zhang et al.

Summary: Cataract is a leading cause of visual impairment, with posterior capsular opacification (PCO) being a common complication of cataract surgery. This study found that bromfenac can effectively inhibit cell migration and epithelial-mesenchymal transition (EMT) to prevent PCO development. Additionally, the researchers developed a drug-eluting intraocular lens (IOL) for sustained release of bromfenac, which showed promising results in preventing PCO in animal models.

NIR-triggered drug delivery system for chemo-photothermal therapy of posterior capsule opacification

Mao Ying-Yan et al.

Summary: Posterior capsule opacification (PCO) is a common complication after cataract surgery, and pharmacological prophylaxis has been proven effective but lacks suitable clinical options. A novel concept of NIR dual-triggered drug release from black phosphorus-based implantable intraocular lens (IOL) for controlled drug release and chemo-photothermal combination therapy of PCO was proposed. The integrated IOL with black phosphorus-loaded doxorubicin showed good transmittance, mechanical properties, NIR dual-triggered drug release behaviors, and excellent photothermal efficacy, demonstrating promising results in inhibiting PCO in in vivo studies.
