Late cardiac events after allogeneic stem cell transplant: incidence, risk factors, and impact on overall survival

Summary: This study aims to investigate the impact of cardiac disease on long term outcomes after allogeneic stem cell transplant (alloSCT). The study found that late cardiac events are relatively common after alloSCT, with the most frequent event being a decline in left ventricular ejection fraction (LVEF). Risk factors for developing a late cardiac event include congestive heart failure prior to alloSCT, decline in LVEF, and cerebral vascular accident. These cardiac events have a negative impact on overall survival.

Summary: Chronic graft-versus-host disease (cGVHD) is a common complication following allogeneic hematopoietic cell transplantation (HCT) in children, and it significantly affects the quality of life of long-term survivors. This study aimed to analyze the occurrence of late effects (LEs) in pediatric HCT survivors and investigate the correlation between cGVHD and subsequent neoplasms (SNs) and nonmalignant LEs (NM-LEs). The findings suggest that patients with cGVHD have a higher cumulative incidence of LEs within 10 years after HCT, and cGVHD-related features are closely associated with the occurrence of NM-LEs.

Further efforts to mitigate recurrence of disease, infections, subsequent neoplasms, cardiovascular disease, and pulmonary disease may be beneficial for this population.
