Pseudoprogression and peritumoral edema due to intratumoral necrosis after Gamma knife radiosurgery for meningioma

Summary: In this study, we investigated whether intratumoral necrosis is a risk factor for peritumoral edema in patients with meningioma after gamma knife radiosurgery. The results showed that the occurrence of postoperative peritumoral edema was significantly higher when intratumoral necrosis occurred after gamma knife radiosurgery. Additionally, meningiomas with intratumoral necrosis experienced temporary volume expansion and edema at 6 months after surgery, characteristics that are consistent with pseudoprogression. These characteristics typically improved at 12 months following gamma knife radiosurgery.

Summary: This study analyzed the long-term volumetric changes of vestibular schwannomas (VS) after LINAC radiosurgery and found that true tumor progression cannot be differentiated from pseudoprogression within the first 5 years. Only after the 6th year, by continuously observing tumor expansion in sequential MRI scans, can the need for salvage treatment be determined.
