Melanoma brain metastases: Biological basis and novel therapeutic strategies

Manali Phadke et al.

Summary: The development of brain metastases is the deadliest complication of advanced melanoma, and recent progress has been made in therapies for melanoma brain metastases (MBM). However, there are lower response durations for intracranial metastases compared to extracranial metastases, possibly due to unique features of the brain microenvironment. Current research focuses on the biology and pathophysiology of melanoma brain metastasis development, as well as the challenges faced in clinical trials and treatment for MBM patients.

Regulation of P-glycoprotein by miR-27a-3p at the Brain Endothelial Barrier

Saba Hammad et al.

Summary: It has been found that miR-27a-3p regulates the expression and activity of P-gp at the blood-brain barrier by mediating Glycogen Synthase Kinase 3 Beta (GSK3β) inhibition and activating Wnt/β-catenin signaling.

miR-27a-3p regulates expression of intercellular junctions at the brain endothelium and controls the endothelial barrier permeability

Rania Harati et al.

Summary: miR-27a-3p has been identified as a positive regulator of tight junction proteins, claudin-5 and occludin, at the brain endothelium. It achieves this by targeting the GSK3 beta gene and activating the Wnt/beta-catenin signaling pathway. This discovery suggests that miR-27a-3p could be a potential therapeutic target for preventing BBB dysfunction in neurological disorders.

Management Strategies for Large Brain Metastases

Nehaw Sarmey et al.

Summary: Brain metastases are common intracranial neoplasms that have a significant impact on individuals and the healthcare system. Surgical resection, stereotactic radiosurgery, and combination therapies have changed the treatment paradigm, although no single treatment has consistently shown effectiveness across patient groups. Considering patient and tumor characteristics may help determine more favorable treatment strategies.

Brain Microvascular Pericytes-More than Bystanders in Breast Cancer Brain Metastasis

Danyyl Ippolitov et al.

Summary: Brain tissue contains a high number of perivascular pericytes, which are known to regulate brain perfusion and play an important role in the neurovascular unit. This review discusses the phenotypes and markers of brain pericytes, as well as the experimental models used to study their function. The crosstalk between pericytes, endothelial cells, and metastatic cells in breast cancer brain metastasis is presented as an example. Future challenges lie in real-time monitoring of pericyte crosstalk to understand the causal events in brain metastasis.

In Vivo miRNA Decoy Screen Reveals miR-124a as a Suppressor of Melanoma Metastasis

Rana S. Moubarak et al.

Summary: Melanoma is a highly prevalent cancer with high metastatic potential. The study found that miR-124a plays a suppressive role in the metastasis of melanoma, especially in brain metastasis. Overexpression of miR-124a does not inhibit tumor growth but specifically controls the metastatic process of melanoma. miR-124a has the potential to be used as a therapeutic agent to impair melanoma metastatic growth and improve patient survival.

How I treat brain metastases of melanoma

Z. Eroglu et al.

Summary: Brain metastases are a common and deadly complication of advanced melanoma. Recent advancements in systemic treatment and radiation therapy have greatly improved the outcomes for patients with brain metastases. Highly active systemic therapy has shown significant benefits, despite concerns about its ability to cross the blood-brain barrier. Factors such as neurologic symptoms, number and size of brain metastases, extracranial disease status, and BRAF mutation status should be considered when determining the best treatment approach.
