Biologics in eosinophilic gastrointestinal diseases

Evan S. Dellon et al.

Summary: Eosinophilic gastrointestinal diseases are a group of conditions categorized based on the location of eosinophilic infiltration in the gastrointestinal tract. Current treatment options include off-label steroids and dietary therapy. Research has identified potential pathways to target in the treatment of these diseases, and some medications such as cendakimab and dupilumab have shown promise in reducing eosinophil count and symptoms. Ongoing phase 3 trials are being conducted to further evaluate the effectiveness of these treatments.

Summary: This study aimed to assess the safety of an investigational formulation of swallowed topical corticosteroids in eosinophilic esophagitis (EoE) patients. The results showed that the participants who received the medication had higher proportions of adverse events (AEs) and adverse events of special interest (AESIs) compared to the placebo group, but most of the events were mild/moderate in severity. Overall, the medication was well-tolerated.

The role of biologics in pediatric food allergy and eosinophilic gastrointestinal disorders

Sayantani B. Sindher et al.

Summary: Continued understanding of the molecular mechanisms of atopic disorders has allowed for the development of targeted biologics. Both food allergy (FA) and eosinophilic gastrointestinal disorders (EGIDs) share similar inflammatory molecular mechanisms and are part of the same atopic disease spectrum. This has led to the investigation of several biologics that target the common key drivers across these diseases. The significant number of ongoing clinical trials and the recent approval of dupilumab for eosinophilic esophagitis highlight the immense potential of biologics in treating FA and EGIDs, emphasizing the need for widespread availability.

Imaging Findings of Eosinophilic Gastrointestinal Diseases in Adults

Camila Lopes Vendrami et al.

Summary: Eosinophilic gastrointestinal (GI) disorders are characterized by abnormal infiltration of eosinophilic cells in the GI tract, including the esophagus, stomach, small intestine, and colon. Clinical presentation depends on the location and depth of eosinophilic infiltration. These disorders should be considered in the differential diagnosis of GI symptoms such as food impaction or dysphagia. Histopathology is the gold standard for diagnosis, but imaging studies can also assist in earlier diagnosis and management. Treatment options include corticosteroids and dietary elimination, and diagnosis often requires a high index of suspicion.

This study aimed to evaluate the differences in microbiota profile between EoE patients and controls through a systematic review. It was found that EoE patients have increased esophageal microbial load and abundance of certain species, such as Haemophilus and Aggregatibacter, while there is a decrease in Firmicutes. Molecular Mechanisms of Eosinophilic Esophagitis

Summary: Food hypersensitivity encompasses diseases driven by specific immune responses to certain foods, with conditions like eosinophilic esophagitis posing challenges in understanding their molecular mechanisms. Researchers aim to classify and identify primary diseases of non-IgE-mediated/mixed food hypersensitivity, exploring distinctive molecular features and discussing potential biomarkers and therapies for eosinophilic esophagitis. The pathogenesis of eosinophilic esophagitis involves complex interactions and signaling at molecular and cellular levels, with alterations in cytokine signaling pathways observed, although a definitive mechanism remains elusive. The position of esophagitis within allergic disorders and eosinophilic gastrointestinal disorders is currently ambiguous.
