Summary: Prosthetic valve endocarditis (PVE) is a serious condition with a high mortality rate. accurate identification of the pathogens is crucial for successful therapy and patient survival. Staphylococcus aureus is the most commonly diagnosed species, and multi-drug resistance pathogens are increasing in prevalence. Blood cultures and echocardiography are commonly used diagnostic methods, but blood culture findings are not always accurate. This review discusses the diversity and virulence potential of PVE-associated pathogens, as well as the challenges in making a microbial diagnosis and offers recommendations for enhanced pathogen identification.

Trends in the incidence and mortality of infective endocarditis in high-income countries between 1990 and 2019

Infective Endocarditis Caused by Staphylococcus aureus After Transcatheter Aortic Valve Replacement

Summary: This study investigated the etiology and clinical characteristics of Staphylococcus aureus infective endocarditis (SA IE) after transcatheter aortic valve replacement (TAVR). It found that SA IE accounted for approximately 25% of all IE cases after TAVR and was associated with high rates of in-hospital and late mortality. The presence of certain features increased the likelihood of SA IE and could guide early antibiotic selection. Surgery at the time of index SA IE was associated with improved outcomes.

Surgical Treatment of Patients With Infective Endocarditis After Transcatheter Aortic Valve Implantation

Summary: This study investigated the clinical characteristics and outcomes of patients with infective endocarditis (IE) after transcatheter aortic valve implantation (TAVI). The study found no significant difference in in-hospital mortality and 1-year all-cause mortality between patients treated with antibiotics alone and patients treated with cardiac surgery and antibiotics. The high mortality in patients with TAVI-IE was strongly associated with patients' characteristics, pathogens, and complications.

Stroke Complicating Infective Endocarditis After Transcatheter Aortic Valve Replacement

Summary: Stroke is a common and potentially disabling complication of infective endocarditis (IE). This study aimed to determine the incidence, risk factors, clinical characteristics, management, and outcomes of patients with definite IE after TAVR complicated by stroke during index IE hospitalization. Analysis of data from the Infectious Endocarditis after TAVR International Registry revealed that 10% of patients had a stroke during IE hospitalization, with higher rates of complications and mortality in stroke patients. Various factors like previous stroke history, residual aortic regurgitation, and valve type were associated with an increased risk of stroke, and surgical treatment did not improve outcomes for these patients.
