Summary: ACTH-producing neuroendocrine neoplasm (NEN) of the thymus is a rare disease with lymph nodes and bones being common metastatic sites. This study reported a case of a 58-year-old woman with intermittent flush and weight loss, who eventually died from infection. The pancreas can serve as a metastatic site for ACTH-producing thymic NEN. EUS-associated procedures may aid in the diagnosis and treatment of pancreatic metastatic NEN.
