Summary: The role of collateral and permeability imaging derived from dynamic contrast material-enhanced magnetic resonance angiography in predicting PH 2 hemorrhagic transformation in acute ischemic stroke was evaluated. The presence of very poor collateral-perfusion status (MAC 0) and the permeable signal on Kep permeability imaging were found to be independently associated with PH 2 hemorrhagic transformation. The specificity of Kep permeability imaging in predicting PH 2 hemorrhagic transformation was 93.8%.
