Vacuum phenomenon as a predictor of kyphosis after implant removal following posterior pedicle screw fixation without fusion for thoracolumbar burst fracture: a single-center retrospective study

Summary: This study reviewed 59 patients with thoracolumbar burst fracture who underwent implant removal following posterior pedicle screw fixation without fusion. Results showed a significant association between the vacuum phenomenon before implant removal and final kyphotic deformity and loss of correction, while other factors were not significantly associated.

The integrity of the posterior ligamentous complex is important in treatment decision-making, and minimally invasive surgery techniques can replace open approaches. Implant removal after stabilization may provide benefits, especially in younger patients.

The results showed that internal fixation removal could improve the quality of life in patients experiencing discomfort. However, in patients without pre-surgery discomfort, the removal of internal fixation had limited or even negative effects, and should not be performed routinely.
