This study analyzed the outcomes of pregnancies after kidney transplantation based on pre-pregnancy estimated glomerular filtration rate-chronic kidney disease categories. It found that pre-pregnancy graft function and hemodynamic adaptation to pregnancy were the main factors affecting adverse outcomes.

Chronic hypertension, pre- or early pregnancy proteinuria, and a gestational fall in serum creatinine of <10% are key predictors of adverse obstetric and renal outcomes. Pregnancy in women with CKD Stages 3-5 advances the need for dialysis or transplantation by 2.5 years.

Summary: A retrospective study compared short-term maternal and fetal outcomes between Dutch kidney transplant recipients undergoing CNI-based immunosuppression and CNI-free immunosuppression. It was found that using CNIs during pregnancy led to higher serum creatinine levels and incidence of low birth weight, but all pregnancies in KTR were classified as high risk regardless of CNI use.
