Early and late outcomes of surgical aortic valve replacement with sutureless and rapid-deployment valves versus transcatheter aortic valve implantation: Meta-analysis with reconstructed time-to-event data of matched studies

Michel Pompeu Sa et al.

Summary: A meta-analysis of reconstructed time-to-event data compared SURD-AVR and TAVI, finding SURD-AVR had lower risk of 30-day mortality and paravalvular leak. However, in the follow-up, TAVI patients had a higher risk of mortality. Surgeons and cardiologists should consider patient factors when selecting between the two approaches.

Five-year outcomes of mild paravalvular regurgitation after transcatheter aortic valve implantation

Summary: This study investigated the impact of mild paravalvular regurgitation (PVR) after transcatheter aortic valve implantation (TAVI) on five-year clinical outcomes. The study found that mild PVR was associated with an increased risk of mortality at five years, particularly mild-to-moderate PVR.

Summary: The sutureless bioprosthesis significantly reduced procedural times during isolated SAVR in both surgical approaches and showed better 1-year outcomes regarding MACCEs, stroke, re-hospitalization, and new-onset atrial fibrillation.

Midterm Outcomes Following Sutureless and Transcatheter Aortic Valve Replacement in Low-Risk Patients With Aortic Stenosis

Victoria Vilalta et al.

Summary: The study compared in-hospital and midterm clinical outcomes following SU-SAVR and TAVR in low-risk patients with aortic stenosis. The results showed that TAVR was associated with improved in-hospital outcomes and valve hemodynamics compared with SU-SAVR. Although similar mortality and stroke rates were observed at 2-year follow-up, the risk of heart failure hospitalization was higher among SU-SAVR patients.

Summary: In a study evaluating low-risk patients with severe aortic stenosis undergoing SAVR, close to half of the patients had major criteria not evaluated in TAVR versus SAVR trials. Clinical outcomes were better than or similar to those predicted by surgical scores in most groups, except for patients requiring CMTV intervention.

Outcomes 2 Years After Transcatheter Aortic Valve Replacement in Patients at Low Surgical Risk

Martin B. Leon et al.

Summary: In low surgical risk patients with symptomatic severe aortic stenosis, the PARTNER 3 trial showed superiority of transcatheter aortic valve replacement (TAVR) over surgery for the primary endpoint at 1 year. However, at 2 years, differences in death and stroke between TAVR and surgery were no longer significant, and TAVR patients had an increased risk of valve thrombosis.

Valve Academic Research Consortium 3: Updated Endpoint Definitions for Aortic Valve Clinical Research

Philippe Genereux et al.

Summary: The Valve Academic Research Consortium (VARC) was established in 2010 to standardize clinical endpoint definitions for aortic valve clinical trials. After several years, a meeting was held involving experts to evaluate and revise specific clinical endpoint definitions, leading to the proposal of new classifications and modifications. These updates will ensure accurate reporting and comparisons in clinical research studies involving aortic valve therapies.

Summary: In a study comparing clinical outcomes and valve durability after 8 years of follow-up in patients with symptomatic severe aortic valve stenosis at low surgical risk treated with either transcatheter aortic valve implantation (TAVI) or surgical aortic valve replacement (SAVR), no significant differences were found in the risk for all-cause mortality, stroke, or myocardial infarction, as well as the risk of bioprosthetic valve failure between the two groups.

Impact of short-term complications of transcatheter aortic valve replacement on longer-term outcomes: results from the STS/ACC Transcatheter Valve Therapy Registry

Suzanne Arnold et al.

Summary: The study showed that complications following TAVR remain common in the first 30 days, with patients experiencing at least one major complication in 21.4% of cases. Among these complications, Stage 3 AKI, stroke, and bleeding were independently associated with significantly increased risk of early death, while moderate/severe PVL and new pacemaker were associated with more modest risk of excess mortality. Additionally, these complications were also linked to less improvement in 1-year health status among survivors.

Eight-year outcomes for patients with aortic valve stenosis at low surgical risk randomized to transcatheter vs. surgical aortic valve replacement

Troels Hojsgaard Jorgensen et al.

Summary: In patients with severe aortic valve stenosis at low surgical risk randomized to TAVI or SAVR, there were no significant differences in the risk for all-cause mortality, stroke, or myocardial infarction, as well as the risk of bioprosthetic valve failure after 8 years of follow-up. However, patients who underwent TAVI had a lower risk of structural valve deterioration compared to those who underwent SAVR.
